Counter-notices
If your material was removed because of a DMCA notice and you believe removal resulted from mistake or misidentification, send a written counter-notice to the designated agent. It must include your physical or electronic signature; identification and former location of the removed material; a statement under penalty of perjury that you have a good-faith belief the material was removed by mistake or misidentification; your name, address, and telephone number; consent to the jurisdiction of the applicable federal district court; and agreement to accept service of process from the original complainant or their agent.
